Our Approach To Support

Care Plus Transitions provides person-centred, outcome-focused support through experienced staff who receive regular training. We offer:

• Emergency placements for crisis situations
• Short-term placements for time-limited support episodes
• Long-term support providing permanence through to age 18
• Group living, solo units, and rural placements with 1:1 or 2:1 staffing

Our History

Care Plus Transitions was founded in October 2010 by social workers and criminal justice specialists. Operating as a not-for-profit Community Interest Company, we reinvest surplus income into service improvements, facility maintenance, post-18 housing, and staff development.

Recruitment

Support Worker starting rate is a minimum of £12.83/hour (£13.20+ with NVQ 3). We offer flexible shifts and genuine career progression — 90% of our managers started out as Support Workers.
