Care Plus Transitions (CPT) was established to develop and extend our services, as part of our commitment to supporting children and young people in care outside their birth families.
The partnership, referred to as "CPTPC", aims to formally support the transition from formal care to unregulated supported accommodation and independence. Following registration, our children's home will serve ages 12–17 within an Ofsted-registered facility.
Transitional Plus Care CIC will continue to provide unregulated supported accommodation to young people aged 16 to 17, with its functions and responsibilities unchanged.
